Complaints Submission Requirements

ConsumerValid Consumer Identity (ID Card/Passport).Must be submitted by the respective consumer
Complaint information (date & time of transaction, amount of transaction, brief chronology of the event)Consumers who apply are checked for validity.

Consumer RepresentativeProof of Identity (ID Card/Passport) of valid Authorizer and Authorized
A Special Power of Attorney from the consumer to the consumer representative stating that the consumer authorizes the individual, agency, or legal institution that represents him to act for and on behalf of the consumer. It has to be submitted with a complete validity period of power attorney and duly stamped.
Other documents, such as a court-issued finding of guardian, that indicate the consumer representative has the right to represent the consumer.
Proof of Identity of the consumer represented and supporting complaint documents required (deposit slip, transfer receipt, bank statements, etc).
